Steps to Apply

Follow the steps below to apply for admissions to our International Program.  Please send needed paperwork to our International Student Program Director Timelle Rosen at trosen@horizon-christian.org or Mail to: Timelle Rosen, 23370 SW Boones Ferry Road, Tualatin, Oregon 97062


Complete the application by providing demographic, medical, and student assessment, and Affidavit of Support information. Scan and email completed application to the Director. Click here to Apply.


Print and distribute three (3) Academic Reference Forms to non-family members. These Reference Forms help to determine a student’s character strengths and weaknesses. Scan and email completed forms to the Director.


Immunization Records are required. Print a copy of Immunization record and take to a doctor to complete.  The Immunization Records must be translated to English. Scan and email the English translated Immunization Forms to the Director.


Official Transcripts are required from the student’s school.  The transcript must be translated into English and contain grade and credit values from grade 8 to present, Transcripts must be mailed directly from the school, in a sealed envelope, to the Director.


Scan and email copy of the identification page of the student’s passport to Timelle Rosen.


Be prepared to pay for the $200 SEVIS I-901 fee. This is required by the United States government and can be paid after the I-20 has been issued from the school. This is not to be sent to the school. Please visit www.fmjfee.com/i901fee for more information.
